Patients using this discount coupon that are enrolled in any state or federally funded healthcare insurance program such as Medicare or Medicaid ("government insurance") may not submit costs or claims for reimbursement for any prescriptions purchased as part of this program to their insurer. Nor may they seek to apply the cost of their prescription toward their deductible or true of out of pocket cost. Patients using this discount coupon must agree that they will purchase any future prescriptions through this program for the full calendar year and will not seek coverage through their government insurance. Patients using this discount coupon must also notify their government insurance about their participation in this program. This program does not include all Amgen products. This patient discount coupon can be used only by eligible United States residents at participating pharmacies in the United States. Patients are responsible for all additional costs and expenses for their prescription after the application of the patient discount coupon has been applied.
